According to the Ohio Department of Health rules for the 2023-2024 school year, students entering as:

7th graders are required to have one dose of both a Tdap and Meningitis (MCV4) vaccine  

12th graders are required to have one or two doses of the Meningitis (MCV4) vaccine.

A Tdap vaccine protects against three diseases: Tetanus, Diphtheria and Pertussis

The MCV4 vaccine protects against four strains of Meningitis (A, C, W, Y).

Please consider scheduling an appointment early in the summer to avoid the back-to-school rush.  August is typically a busy time when many parents are getting their children up to date on their vaccinations before the school year starts. Appointments can be scheduled by calling the Putnam County Health Department at 419-523-5608. Area pharmacies and medical offices also provide the vaccines.

Please send a copy of your child’s immunization record to the HS office or have your
student present it on their first day of school.
